(Under Miscellaneous Services CGI, New York is offering following Services: Attestation of Documents, Attestation of Power of Attorney, Affidavit for Child’s Passport to be issued in India, Marriage Certificate, Life Certificate, Birth Certificate, Police Clearance Certificate, NRI Certificate, NORI I, NORI III, Death Documents Attestation -Transportation of Ashes/Mortal Remains to India, Attestation of will, Registration of Birth etc.)

GENERAL INFORMATION

There is no need to take an appointment. Applicants can visit the Consulate on any working day.

For submission of documents: 9:15 am - 11:15 am.

Delivery of documents: 11:30 am - 13:00 pm.

The service will be on first come first serve basis. Before, approaching the Consular Office to seek required service, applicant must be ready with complete application form and other required documents, as mentioned in the Consulate website. This is important to render better and quick service to the applicants. Following is the Consular jurisdiction of the Consulate: Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont. Applicants only from these states may apply at the Consulate General of India, New York. Please apply strictly as per your jurisdiction for which you have verifiable proof of address.

Appostile Convention: The Hague Convention, to which India and USA are signatories, abolishes the requirement of legalization for Foreign Public Documents. With the certification by the Hague Convention 'Apostille', the document is entitled to recognition in India and vice versa and no certification by the Consulate of India is required. The Convention applies to public documents which have been executed in the territory of one Contracting State and which have to be produced in the territory of another Contracting State.

The Consulate does not certify genuineness of any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, educational documents/diplomas, marriage certificate and Driving License etc.) are required to be presented to authorities in USA, these should be apostilled in India. For details, please check the website “Legalization of Documents” at http://www.mea.gov.in/apostille.htm   . However, for the ease of Indian nationals, Consulate/Consulates does “True Copy Attestation Only” of the documents issued by authorities in India, which does not mean certifying the genuineness of the document.

It is not mandatory that the apostilled documents should be attested by the Consulate officials before presenting it to authorities in India.

INSTRUCTIONS :

A. Local documents like bank statement, salary statement, employment letter of the applicant, Driving License etc. need to be Notarized by the applicant and only signature of the applicant will be attested. On each document to be attested, applicant will mention “I TAKE FULL RESPONSIBILITY FOR THE CONTENTS OF THE DOCUMENT”. After that applicant will put his full name and signature (as per passport) on each document submitted for attestation.

B. For attestation of Indian passport, Driving License, Pan Card, Adhar Card, Voter ID, Educational documents etc., applicant may submit original and a copy of the same. Documents which are not apostilled from India will not be authenticated by Consulate. Such documents will only be attested as ‘True Copy Attestation Only”. On each document to be attested, applicant will state that “I TAKE FULL RESPONSIBILITY FOR THE CONTENTS OF THE DOCUMENT” and write full name and sign, as in the passport.

C. List of documents valid as a proof of Address/Residence (Copy of any one of following):

• US Driving License OR
• Utility Bill OR
• Lease Agreement OR
• State Identity Card OR
• Mortgage Deed

D. Proof of US Visa Status (Copy of any one of the following):

• Visa page of Passport (i.e. H1B, H4, L1, L2 etc.) OR
• Green Card OR
• Employment Authorization Document (Work Permit) OR
• I-797, I-140 or I-20 (If approval is pending, attach a handwritten note detailing the efforts being taken to regularize status)

E. Fees :

• Total fee mentioned against each service include fee of US$ 2/- (two) towards Indian Community Welfare Fund (ICWF). This is only one time fee for first copy of attestation/first copy of certificate. ICWF will not be charged for subsequent copy (if needed). For subsequent copy, applicant has to pay only required fees for services (excluding ICWF portion).
• Applicant who opted for “In Person” mode: Required fee may be paid in Cash only.
• Applicant who opted “Post/Mail” mode : Required fee may be paid by ‘Money Order/Cashiers’ Cheque in favor of “Consulate General of India, New York”.
• Personal Cheques, Credit Cards, Debit Cards Or Other Banking Instruments Are Not Accepted.

This will give you a detail information about documents, fee, and forms needs to be filled in respect of particular service.

H. Online Misc. Service Application Form: You will find link of Misc. Service application form in Misc. Service Tab, fill and submit Misc. Service form online and take a printout of this submitted form. Submit physical copy of Misc. application form along with all other documents, fees to the Consular Wing, CGI, New York by selected Submission Mode (i.e. In Person/Mail).
